Qualifications

• Requires a relevant degree in Social Work, Psychology, Counseling, or related field.
• 3–5 years of experience in case management or social support.
• Proven experience working remotely/tele-support.
• Excellent verbal and written communication with active listening.
• Proficient with case management software and virtual tools.
• Ability to work independently, maintain confidentiality, and manage time.

Responsibilities

• Conduct remote needs assessments for clients seeking social support.
• Develop individualized case management plans tailored to client goals.
• Provide counseling, guidance, and emotional support to clients and families.
• Facilitate referrals to social services and community resources.
• Monitor progress and update case plans as needed.
