Placements24 is seeking a compassionate Remote Social Services Coordinator to manage and deliver vital support services for the Centurion community. This fully remote role focuses on coordinating client care, connecting individuals with resources, and advocating for needs through virtual channels.

You will work within a forward-thinking organization that values remote collaboration and strives to broaden access to essential services, delivering meaningful impact while maintaining confidentiality

Qualifications

Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
Experience in social work, case management, or community support roles.
Knowledge of social services, mental health resources, and community networks.
Strong virtual communication, interpersonal, and active listening skills.
Familiarity with virtual tools and maintaining confidentiality remotely.

Responsibilities

Coordinate the delivery of social services to clients through virtual means (phone, video, email).
Assess client needs and connect them with community resources and support programs.
Develop and maintain case management plans and track progress.
Provide guidance and emotional support to clients and families.
Maintain confidential electronic client records and documentation.
Collaborate with internal teams and external providers in a remote setting.
