Placements24 is seeking an experienced Social Care Coordinator to join a fully remote team serving the George region. You will coordinate a range of social care services, liaising with clients, providers and community resources to ensure timely, effective care paths.

The role requires strong organization, excellent virtual communication, and a commitment to improving community well-being through remote delivery.

Qualifications

Bachelor's degree in Social Work or Psychology or related field.
Minimum 3–5 years of experience in social care management or coordination.
Proven ability to develop and manage effective care plans.
Excellent virtual communication and interpersonal skills.

Responsibilities

Oversee the delivery of social care programs and services, ensuring compliance with relevant standards and regulations.
Conduct client assessments and develop personalized care plans tailored to individual needs and circumstances.
Liaise with external agencies, healthcare professionals, and support networks to facilitate comprehensive care.
Monitor client progress and adjust care plans as necessary to ensure optimal outcomes.
Provide guidance and support to a team of remote community support staff.
Maintain accurate client records and prepare regular reports on service delivery and client outcomes.
