Placements24 is seeking a Senior Community Support Worker to work remotely across South Africa, coordinating and delivering essential support services from a home office. You will mentor junior staff, contribute to program development, and ensure high standards of care in diverse community settings.

Qualifications

• A relevant tertiary qualification in Social Work, Psychology, Community Development, or a related field.
• Minimum 5 years of proven experience in community support, social work, or a similar role.
• Demonstrated experience in case management and developing individualized support plans.
• Proficiency in using virtual communication and collaboration tools (e.g., Zoom, Microsoft Teams, Slack).

Responsibilities

• Develop, implement, and manage individualized support plans for clients, ensuring a holistic approach to their needs.
• Provide direct support and guidance to clients, addressing social, emotional, and practical challenges they may face.
• Facilitate access to community resources, healthcare services, and educational opportunities for clients.
• Conduct regular assessments and evaluations to monitor client progress and adjust support strategies as needed.
• Collaborate with external agencies, healthcare providers, and other stakeholders to ensure comprehensive care coordination.
• Maintain accurate and confidential client records, adhering to all privacy regulations and organizational policies.
• Mentor and supervise junior community support workers, providing training and ongoing professional development.
• Organize and lead virtual community engagement activities and support groups to foster connection and reduce isolation.
• Contribute to the development and improvement of service delivery models and best practices.
• Act as a primary point of contact for clients and their families, offering reliable and empathetic support.
