Job summary

Placements24 is seeking a compassionate Remote Residential Support Worker to assist individuals in supported living environments entirely through remote channels. You will coordinate care, monitor progress, and connect residents with services from your own location, ensuring safety, dignity, and independence through technology.

The role requires excellent virtual communication, strong organization, and the ability to build rapport with residents and their networks to empower independent living.

Qualifications

• Qualified in healthcare, social work, community services, or related field.
• Proven experience in residential support, aged care, disability services, or similar caregiving role.
• Experience communicating empathetically through virtual platforms.

Responsibilities

• Conduct remote check-ins and assessments to monitor well-being and safety.
• Develop and implement individualized support plans with residents and networks.
• Coordinate with healthcare providers, family, and services for comprehensive care.
• Provide guidance for daily living, health management, and social engagement remotely.
• Document progress and incidents in the online case management system.
