The position emphasizes assessing client needs, developing care plans, and liaising with healthcare and social service providers. A master’s in social work and SACSSP registration are required, with at least three years of experience

Qualifications

• Master's degree in Social Work or equivalent.
• Current SACSSP registration.
• Minimum 3 years of experience in case management or direct client service.

Responsibilities

• Conduct remote client assessments via video conferencing and phone calls.
• Develop, implement, and monitor individualized case management plans.
• Coordinate access to community resources, healthcare, and social services.
• Maintain accurate and confidential electronic client records.
• Provide ongoing support, advocacy, and crisis intervention to clients remotely.
• Liaise effectively with external agencies and service providers via digital channels.
