Job summary

Placements24 is seeking a compassionate Case Manager specialized in Elderly Care for a fully remote role. You will assess needs, develop individualized care plans, and coordinate services with families, healthcare providers and community supports to promote seniors’ wellbeing and independence.

Qualifications

• Bachelor's degree in Social Work, Gerontology, Psychology, or related field.
• Minimum of 3 years of experience in case management, preferably in elder care.
• Strong understanding of elderly needs and local resources.
• Excellent assessment, planning, intervention, and evaluation skills.
• Proficiency with case management software, telehealth platforms, and productivity tools.
• Excellent communication, interpersonal and problem-solving abilities.

Responsibilities

• Conduct comprehensive needs assessments for elderly clients, evaluating physical, emotional, and social well-being.
• Develop, implement, and monitor personalized care plans, ensuring client goals are met.
• Coordinate services with healthcare professionals, caregivers, and community support agencies.
• Provide regular updates and communication to clients and their families regarding care progress.
• Advocate for clients' needs and rights, ensuring they receive high-quality care.
• Maintain accurate and confidential client records, case notes, and documentation in compliance with standards.
