Placements24 is seeking a Remote Community Health Navigator to support individuals and families across South Africa from home. You will link patients with healthcare providers, explain health conditions, and help people navigate insurance, services, and resources while upholding health equity.

You will work independently, using telehealth tools to maintain confidential records and collaborate with care teams.

Qualifikationen

• Bachelor's degree in Public Health, Health Sciences, Social Work, Nursing or related field.
• Minimum 3 years experience in healthcare or social services with client-facing duties.
• Proven health navigation, patient advocacy or community outreach experience.
• Strong understanding of healthcare systems, insurance and medical terminology.
• Excellent communication, active listening and empathy, with virtual engagement ability.
• Ability to manage a caseload and use telehealth tools and EHR systems.

Aufgaben

• Provide one-on-one remote support to help clients understand and manage health conditions.
• Navigate healthcare system: schedule appointments, explain benefits, access records.
• Connect clients with community resources and social services.
• Educate on preventive health, healthy lifestyle, and treatment adherence.
• Conduct virtual needs assessments and tailor support per client.
• Maintain confidential client records via secure telehealth platforms and EHR.
• Collaborate with providers, social workers and other professionals for coordinated care.
• Develop and deliver health topics via webinars, portals and digital tools.
• Advocate for clients within the healthcare system to ensure their rights.
• Stay updated on policies, resources and best practices in remote patient support.
