Job summary

Placements24 is seeking a proactive Community Health Navigator to support individuals remotely in understanding and accessing health and social services. You will guide clients through complex systems, advocate for their needs, and coordinate with providers to ensure timely care.

The role focuses on equity in access, practical remote engagement, and ongoing follow-ups to ensure clients receive the services they require in a fully remote setting.

Qualifications

• Experience in client advocacy or case management is preferred.
• Strong understanding of health care systems and social services.
• Excellent verbal and written communication with empathy.

Responsibilities

• Assist clients remotely to identify health and social service needs.
• Provide information on available health services, insurance options, and community resources.
• Guide clients through scheduling appointments and completing applications.
• Liaise between clients, healthcare providers, and social service agencies.
• Advocate for equitable access to care and services.
• Maintain knowledge of local health and social service providers.
• Document interactions and outcomes in the case management system.
• Conduct virtual follow-ups to ensure services are received.
• Identify barriers to care and provide feedback for program improvement.
• Participate in virtual team meetings and training sessions.
