Working from a home office, you will guide clients through healthcare systems, schedule appointments, and advocate for equitable access using telehealth tools.

The role requires a Bachelor’s degree in Public Health or related field and at least three years in community health or patient advocacy.

Qualifications

• Bachelor’s degree in Public Health, Social Work, Nursing, or related field.
• Minimum of three years of experience in community health, patient advocacy, or health navigation.
• Proficiency with telehealth platforms, virtual tools, and electronic health records.

Responsibilities

• Conduct virtual needs assessments to understand clients’ health service requirements and barriers.
• Provide telephonic and online guidance on navigating healthcare systems, insurance, and programs.
• Assist clients in scheduling appointments with healthcare providers and accessing medical services.
• Educate community members on preventive health practices and available wellness resources via digital channels.
• Maintain accurate electronic health records and case management documentation.
• Collaborate remotely with healthcare providers, community organizations, and internal teams to ensure comprehensive client support.
