Job summary

Placements24 seeks a Community Health Navigator to empower residents of Mossel Bay and surrounding Western Cape region. This fully remote role links clients with healthcare services, educates on disease management, and coordinates care plans from home to improve local health outcomes.

The ideal candidate has a Health Sciences background, 2+ years in community health or patient navigation, and strong communication and empathy to support diverse communities remotely.

Qualifications

• Bachelor’s degree or equivalent in Health Sciences, Public Health, Social Work or related field.
• At least 2 years in community health, patient navigation, or similar role.
• Knowledge of Mossel Bay health services and social support systems is advantageous.
• Excellent verbal and written communication with ability to explain health information clearly.
• Strong empathy, patience and active listening in a remote setting.

Responsibilities

• Assist individuals in navigating the healthcare system to access services.
• Provide health education on chronic disease management and preventive care.
• Help clients understand diagnoses and treatment plans.
• Coordinate appointments and follow-ups with healthcare providers.
• Connect clients with social support services and community resources.
• Maintain confidential records and track progress.
• Collect data on service utilization and outcomes.
• Advocate for clients’ health needs with providers and systems.
