Qualifications

• Registered Nurse (RN) or Social Worker (MSW) or equivalent health-related degree.
• Minimum 3–5 years in case management, community health, or clinical practice.
• Experience managing a caseload remotely and using telehealth tools.

Responsibilities

• Conduct comprehensive remote assessments via telehealth.
• Develop individualized care plans and coordinate referrals.
• Schedule medical appointments and follow-ups.
• Connect clients with community health resources and social services.
• Provide ongoing support, education, and advocacy to clients and families.
• Monitor progress, document interventions, and evaluate outcomes.
• Maintain confidential electronic health records and collaborate with providers.
