Job summary

Placements24 is seeking a Remote Case Manager to coordinate support services for individuals involved in community health initiatives. The role is fully remote and targets residents around Centurion in Gauteng, SA.

You will assess needs, develop care plans, liaise with providers, and maintain confidential records while working across digital platforms to ensure timely and effective service delivery.

Qualifications

• 3+ years' experience in case management, social support, or community health coordination.
• Experience working remotely using digital tools.
• Understanding of South Africa healthcare landscape, especially Centurion region.

Responsibilities

• Assess client needs and develop individualized case management plans.
• Coordinate access to community health and social services.
• Monitor progress and adapt care plans as needed.
• Maintain confidential client records with privacy compliance.
• Liaise with healthcare providers and social services.
• Provide remote support and information to clients and families.
• Document interactions and outcomes meticulously.
• Participate in virtual meetings and case conferences.
