Placements24 is seeking a dedicated Remote Community Health Navigator to connect individuals with essential health services from a home office. This fully remote role serves clients across Gauteng, with a focus on navigating the healthcare system, arranging appointments, and linking people to community resources.

You will need a public health or related degree, at least three years in healthcare or social services, and strong skills in virtual communication and EHR.

Qualifications

• Degree in Public Health, Nursing, Social Work, or related field.
• Minimum 3 years in healthcare, community health or social services.
• Experience in patient navigation or case management.
• Strong knowledge of healthcare landscape and community resources.
• Proficient with virtual tools and EHR systems.
• Exceptional remote communication and interpersonal skills.
• Ability to manage a caseload and work autonomously.
• Cultural competency with diverse populations.
• Reliable internet and dedicated home office.

Responsibilities

• Assist community members in understanding their health needs and options.
• Guide clients through the healthcare system to access services like doctor appointments, specialists, and diagnostic tests.
• Connect individuals with community resources for social support, nutrition, and mental health services.
• Provide health education and information on preventive care and chronic disease management.
• Conduct needs assessments through virtual channels (phone, video calls, online forms).
• Maintain accurate and confidential client records in a secure digital environment.
• Build and maintain relationships with healthcare providers and community organizations.
• Follow up with clients to ensure they have connected with services and address any barriers.
• Document all interactions, referrals, and client progress meticulously.
• Advocate for clients\' needs within the healthcare and social service systems.
