Job summary

Placements24 is seeking a Remote Community Health Navigator to bridge gaps in healthcare access for Stellenbosch residents. The role focuses on guiding individuals through health systems, offering information on resources, and coordinating care remotely.

You will work autonomously in a fully remote environment, collaborating with providers and community orgs to improve health outcomes and ensure equitable access to services.

Qualifications

• Bachelor's degree in Public Health, Health Sciences, Social Work, or a related field.
• Minimum of 2 years of experience in health navigation, community health work, or a similar role.
• Strong knowledge of the healthcare system and available community health resources.
• Excellent virtual communication and interpersonal skills, with empathy and cultural competence.
• Proven ability to work autonomously and manage time effectively in a remote environment.
• Proficiency with telehealth platforms and electronic health record systems.

Responsibilities

• Assist community members in understanding health conditions, treatment options, and preventative care strategies.
• Guide clients through the process of accessing healthcare services, including appointments and insurance enrollment.
• Provide information on available health resources, support groups, and community health programs.
• Conduct virtual needs assessments to identify barriers to healthcare access and develop tailored action plans.
• Maintain accurate and confidential client records and document all interactions and referrals.
• Act as a liaison between clients and healthcare providers to ensure effective communication and follow-up.
