Responsibilities

• Connect clients with community resources, healthcare services, and educational opportunities.
• Offer ongoing emotional support and guidance via phone, video calls, and secure messaging.
• Assist clients in navigating government benefits, insurance claims, and other administrative tasks.
• Advocate for clients' rights and needs within various service systems.
• Maintain detailed and confidential electronic client records.
• Collaborate with a network of remote healthcare providers, therapists, and support organizations.
• Facilitate virtual group sessions and workshops for clients and their families.
• Stay updated on best practices and resources in disability support and remote service delivery.
