Job summary

Placements24 seeks a dedicated Disability Support Coordinator to empower clients across South Africa from a fully remote team. You will assess needs, coordinate services, advocate for rights, and support independence, with a focus on the Port Elizabeth region.

You will work autonomously, leverage virtual tools, and collaborate with government and non-profit partners to deliver person-centred care and measurable outcomes in a remote, flexible environment.

Qualifications

• Bachelor's degree in Social Work, Psychology, Disability Studies, or related field.
• Minimum 4 years of experience in disability support services, case management, or similar role.
• In-depth knowledge of disability legislation, support systems, and resources in South Africa.
• Proven ability to develop and implement effective support plans.
• Excellent communication, interpersonal, and advocacy skills.
• Strong organizational and time management skills with the ability to manage multiple cases remotely.

Responsibilities

• Assess the needs of individuals with disabilities and develop personalized support plans.
• Coordinate and facilitate access to a range of support services, including healthcare, education, employment, and community inclusion.
• Act as a primary point of contact and advocate for clients, ensuring their rights and needs are met.
• Build and maintain relationships with government agencies, non-profits, and service providers.
• Monitor client progress and review support plans for effectiveness.
• Provide information and guidance on available resources and entitlements.
• Maintain accurate client records and prepare reports on service delivery and outcomes.
• Participate in virtual team meetings, case conferences, and professional development sessions.
• Stay informed about disability legislation, policies, and best practices.
• Offer emotional support and empower clients to achieve independence and quality of life.
