Job summary

Placements24 is seeking a proactive Disability Support Officer in Centurion, Gauteng. This remote role focuses on empowering individuals with disabilities to live independently, access resources, and participate in the community.

You will work with clients, families, and networks to coordinate care and support services. The ideal candidate has a relevant tertiary qualification and at least 4 years of direct disability support experience, with strong communication and problem-solving skills.

Qualifications

• Requires tertiary qualification in a relevant field such as Social Work, OT, Disability Studies or Psychology.
• Ability to manage a caseload and coordinate services for remote clients.
• Strong communication, advocacy and person-centred support skills.

Responsibilities

• Develop individualized support plans aligned with client goals.
• Assist with daily living, vocational training, and community participation.
• Connect clients with community services and healthcare providers.
• Educate clients and families on resources and adaptive strategies.
• Monitor progress, document interventions, and report outcomes.
• Advocate for clients’ rights across settings.
• Facilitate use of assistive technology to promote independence.
• Conduct risk assessments and develop safety plans with clients.
• Organize community-based activities and foster social inclusion.
• Maintain up-to-date knowledge of disability best practices.
