Qualifications

• Bachelor's degree in Social Work or related field.
• Minimum 5 years of case management experience in disability services.
• Knowledge of disability legislation, resources, and SA support systems.
• Proven ability to develop and implement person-centered care plans.
• Experience working remotely and managing a caseload.

Responsabilités

• Conduct thorough assessments and develop individualized support plans.
• Coordinate and facilitate access to healthcare, education, and community resources.
• Provide ongoing case management and adjust plans as needed.
• Advocate for clients and promote autonomy and rights.
