Qualifications

• A relevant qualification in Disability Studies, Social Work, Community Development, or a related field.
• Proven experience working with individuals with various disabilities.
• Comprehensive understanding of different types of disabilities and person-centered support approaches.
• Excellent communication, interpersonal, and active listening skills.
• Ability to work independently and manage time effectively in a remote capacity.
• Proficiency in documentation and record-keeping.
• Strong problem-solving skills and the ability to respond effectively to challenging situations.
• Commitment to promoting inclusion, dignity, and respect for all individuals.
• A proactive and adaptable approach to client support.

Responsibilities

• Develop and implement individualized support plans tailored to the specific needs and goals of each client.
• Provide direct assistance with daily living activities, personal care, and household tasks as required.
• Support clients in accessing community resources, engaging in social activities, and pursuing educational or vocational opportunities.
• Assist clients in developing life skills, promoting independence, and fostering self-advocacy.
• Build and maintain positive, respectful relationships with clients, their families, and support networks.
• Monitor client progress, document interventions, and report on outcomes to the care team.
• Ensure a safe and supportive environment, adhering to all health and safety regulations.
• Facilitate effective communication between clients, families, and other service providers.
• Provide emotional support and encouragement to clients.
