Placements24 is seeking a compassionate Disability Support Case Manager to join a fully remote team serving clients across South Africa, with a focus on the Pretoria region. You will assess needs, develop ISPs, and connect clients to healthcare, education, housing and employment resources, while advocating for rights and independence.

Ideal candidates have a relevant degree and at least 3 years of disability case management experience, with strong communication and organizational skills.

Formación

• Bachelor's degree in Social Work or related field.
• Minimum 3 years of disability case management experience.
• Strong knowledge of SA disability resources and services.
• Excellent assessment, planning, and problem-solving skills.
• Empathetic, patient and client-centered approach.

Responsabilidades

• Conduct comprehensive needs assessments for individuals with disabilities.
• Develop and review individualized support plans with clients and families.
• Coordinate access to healthcare, therapy, education, employment and housing services.
• Advocate for clients, ensuring rights and needs are met.
• Maintain accurate case notes and documentation in the client management system.
