Job summary

Placements24 is seeking a compassionate Remote Social Support Facilitator to join our Community & Social Care team. This fully remote, full-time position supports individuals and families by connecting them with resources, guidance, and ongoing virtual support.

The ideal candidate holds a Bachelor's degree in Social Work, Psychology, or Sociology, with at least 3 years of remote client experience, strong digital communication skills, and the ability to work independently while maintaining high

Qualifications

• Demonstrated ability to provide remote client support and build rapport virtually.
• Excellent verbal and written communication skills with proficiency in digital platforms.
• Strong organizational skills and ability to manage a caseload independently.
• Familiarity with community resources and social service systems.
• Ability to exercise discretion, empathy, and professional judgment.

Responsibilities

• Provide remote social support and guidance to clients through various digital communication channels (phone, video calls, email).
• Facilitate client access to essential community resources, government services, and support programs.
• Conduct remote needs assessments and develop personalized support plans.
• Educate clients on self-advocacy, coping strategies, and available resources.
• Maintain detailed and confidential electronic records of client interactions, progress, and service provision.
• Collaborate with other professionals and agencies to ensure a holistic approach to client care.
• Monitor client progress and follow up regularly to ensure ongoing support and address any emerging needs.
• Develop and deliver virtual workshops or information sessions on relevant topics.
• Stay updated on best practices, policies, and resources within the Community & Social Care sector.
• Uphold the highest standards of ethical conduct and client confidentiality in all remote interactions for South Africa.
• Act as a compassionate advocate for clients, ensuring their voices are heard and needs are met.
