About the Role

Our client is a forward-thinking healthcare organization committed to expanding access to essential services, and they are actively seeking a dedicated Remote Community Health Navigator to join their virtual team. This position is a fully remote opportunity, allowing you to contribute impactful work from the comfort of your home, supporting individuals within the wider community who may face barriers to healthcare access. You will act as a crucial link, connecting individuals with health resources, educational materials, and community support services. The ideal candidate possesses excellent communication skills, a deep understanding of healthcare systems, and a passion for advocacy, serving populations that may include those in areas like Mossel Bay, Western Cape , but not limited to it, as the role is entirely remote.

Key Responsibilities

• Conduct virtual outreach to individuals and families to assess their healthcare needs and identify barriers to access.


• Provide information and guidance on available health services, insurance options, and community resources.


• Assist clients in navigating complex healthcare systems and scheduling appointments virtually.


• Educate clients on preventive health measures, disease management, and healthy lifestyle choices.


• Connect clients with appropriate specialists, primary care providers, and social support services.


• Maintain detailed and confidential client records using secure, HIPAA-compliant digital platforms.


• Collaborate with a remote team of healthcare professionals, social workers, and other navigators to share best practices and support clients holistically.


• Develop and distribute educational materials on various health topics through digital channels.


• Track client progress and outcomes, reporting on key performance indicators to management.


• Stay updated on community health trends, available resources, and changes in healthcare policy.

Requirements

• A Bachelor's degree in Public Health, Social Work, Health Administration, or a related field is preferred.


• Previous experience in healthcare, social services, community outreach, or a similar role.


• Demonstrated understanding of healthcare systems, insurance, and community resources.


• Exceptional verbal and written communication skills, with the ability to convey complex information clearly and empathetically.


• Proficiency in using virtual communication tools (e.g., Zoom, Microsoft Teams) and digital case management software.


• Strong organizational skills and the ability to manage a remote caseload independently.


• A reliable internet connection and a dedicated, quiet workspace conducive to remote work.
