Placements24 seeks a Community Health Navigator to empower residents of Mossel Bay and surrounding Western Cape region. This fully remote role links clients with healthcare services, educates on disease management, and coordinates care plans from home to improve local health outcomes.

The ideal candidate has a Health Sciences background, 2+ years in community health or patient navigation, and strong communication and empathy to support diverse communities remotely.

Qualifications

• Bachelor’s degree or equivalent in Health Sciences, Public Health, Social Work or related field.
• At least 2 years in community health, patient navigation, or similar role.
• Knowledge of Mossel Bay health services and social support systems is advantageous.
• Excellent verbal and written communication with ability to explain health information clearly.
• Strong empathy, patience and active listening in a remote setting.

• Excellent verbal and written communication skills, with the ability to explain complex health information clearly.
• Strong empathy, patience, and active listening skills.
• Proficiency in using virtual communication platforms (e.g., Zoom, Teams) and electronic health record systems.
• Ability to work independently, manage time effectively, and maintain motivation in a remote setting.

• Fluency in at least one local indigenous language is a plus.
