Placements24 is seeking a proactive Community Health Navigator to support individuals remotely in understanding and accessing health and social services. You will guide clients through complex systems, advocate for their needs, and coordinate with providers to ensure timely care.

About the Role

Key Responsibilities

• Assist clients in identifying their healthcare and social service needs through remote consultations.
• Provide information and education on available health services, insurance options, and community resources.
• Guide clients through the process of scheduling appointments, completing applications, and navigating eligibility requirements.
• Act as a liaison between clients, healthcare providers, social service agencies, and other relevant entities.
• Advocate for clients' needs to ensure equitable access to care and services.
• Develop and maintain knowledge of local and regional health and social service providers.
• Document client interactions, progress, and outcomes accurately in the designated case management system.
• Conduct virtual follow-ups to ensure clients are receiving the services they need and are satisfied.
• Identify systemic barriers to care and provide feedback to program managers for service improvement.
• Participate in virtual team meetings, training sessions, and contribute to program development.

Requirements

• A relevant qualification in Public Health, Social Work, Community Development, or a related field is preferred.
• Proven experience in a role involving client advocacy, case management, health navigation, or community outreach.
• Strong understanding of the healthcare system, social services, and the challenges individuals face in accessing them.
• Excellent verbal and written communication skills, with the ability to explain complex information clearly and empathetically.
• Proficiency in using virtual communication tools (video conferencing, phone, secure messaging) and case management software.
• Strong organizational skills and the ability to manage multiple client cases concurrently.
• Ability to work independently, manage time effectively, and maintain motivation in a remote work environment.
• A reliable high-speed internet connection and a dedicated, professional home workspace are required.
