Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or related field.
• Previous experience in case management or similar client support role.
• Proficiency with digital communication tools and standard office applications.
• Excellent communication, empathy and problem-solving skills for remote work.

Responsibilities

• Manage a caseload of clients and develop individualized support plans remotely.
• Provide counseling, advocacy and resource navigation via phone, video, and secure messaging.
• Monitor client progress and adherence to plans and intervene as needed.
• Maintain accurate and confidential electronic client records and documentation.
• Collaborate with remote team members and external service providers to ensure comprehensive care.
