Placements24 is seeking an experienced Remote Case Manager to join our virtual social services team serving clients in the Benoni region. You will manage a caseload remotely, assess needs, develop personalized support plans, and connect clients with community resources from your home office.

The role requires a bachelor’s degree in a human services field and at least 3 years of case management experience, with proficiency in Zoom or Teams and secure record keeping.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or related field.
• Minimum of 3 years of experience in case management or social work.
• Proficiency with case management software and virtual tools (e.g., Zoom, Teams).

Responsibilities

• Conduct comprehensive client needs assessments via video calls and phone consultations.
• Develop, implement, and monitor individualized case management plans.
• Provide supportive counseling and guidance to clients to help them achieve their goals.
• Research, identify, and connect clients with appropriate community resources and social services.
• Maintain detailed and accurate client records within the agency's database system.
• Collaborate virtually with support staff, external agencies, and service providers.
