Job summary

Placements24 is seeking a highly organized Remote Case Management Specialist to join our virtual operations. This role focuses on delivering comprehensive support to clients remotely, managing a caseload, developing personalized plans, and coordinating with service providers to ensure client success.

You will work from home with excellent communication skills, use case management software and secure messaging, and maintain confidential records in line with standards.

Qualifications

• Bachelor's degree in Social Work or related field; relevant certification considered.
• Experience with remote clients and telehealth/remote support.
• Strong assessment, planning, intervention, and evaluation skills.

Responsibilities

• Manage a diverse caseload, conduct initial assessments, and develop personalized plans.
• Provide ongoing support and resource navigation via phone, video calls, and secure messaging.
• Coordinate services with external agencies and healthcare providers to meet client needs.
• Monitor progress, evaluate effectiveness, and adjust plans as needed.
• Maintain confidential client records in compliance with organizational standards.
