Job summary

Placements24 is seeking a dedicated Remote Case Manager to join the virtual team. You will assess client needs, develop tailored intervention plans, and connect clients with resources from your home office, supporting the North West region and beyond.

This remote role emphasizes compassionate client support, confidential record-keeping, and collaboration with external agencies to improve outcomes across various communities.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• 3+ years of experience as a Case Manager or in social services.
• Proficiency with virtual communication tools (Zoom, Teams) and electronic case management software.
• Strong assessment, planning, intervention, and evaluation skills.

Responsibilities

• Conduct client needs assessments via secure video conferencing and phone calls.
• Develop, implement, and monitor individualized case plans to address client goals.
• Provide ongoing support, counseling, and resource navigation to clients.
• Coordinate services with external agencies, healthcare providers, and community organizations.
• Maintain accurate, timely, and confidential client records using the organization's system.
