Job summary

Placements24 is seeking a dedicated Remote Case Manager to provide essential support services to individuals and families across communities. This fully remote role requires assessing client needs, developing personalized support plans, and connecting clients with appropriate resources.

You will manage a caseload remotely, using digital tools for documentation and communication, ensuring timely, coordinated services and advocacy that improve client outcomes.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Proven experience in case management or social services delivery.
• Strong understanding of community resources and social welfare systems.
• Excellent assessment, counseling, and crisis intervention skills.
• Proficiency in using virtual communication platforms and case management software.
• Ability to work independently in a remote setting.

Responsibilities

• Conduct comprehensive client assessments to identify needs, strengths, and challenges.
• Develop, implement, and monitor individualized service and support plans.
• Coordinate services with other healthcare providers, social agencies, and community resources.
• Provide ongoing support, advocacy, and guidance to clients and their families.
• Maintain accurate and up-to-date client records and documentation using electronic systems.
