Placements24 is seeking a highly motivated Remote Case Manager to join its expanding community support services. This fully remote role lets you manage a caseload from home, coordinate services, and advocate for clients across agencies.

Responsibilities include needs assessments via phone/video, developing action plans, coordinating healthcare, housing, employment, and social programs, and maintaining confidential records in an electronic system.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Minimum of 3 years in case management or similar community services.
• Experience with diverse populations and complex needs.

Responsibilities

• Conduct client needs assessments remotely via phone or video.
• Develop, implement, and monitor individualized case plans.
• Coordinate access to healthcare, housing, employment, and social programs.
• Provide ongoing support, counseling, and crisis intervention online.
• Maintain accurate client records in the electronic system.
• Advocate for client needs with external agencies.
