Placements24 is seeking a dedicated Remote Case Manager to join its Community Support team. This fully remote role serves the Richards Bay, KwaZulu-Natal region, assessing client needs and coordinating services to achieve optimal outcomes from the comfort of home.

You will manage caseloads, develop personalized care plans, and collaborate with health providers, social services, and families while using digital tools to document progress and ensure continuity of care.

Formación

• Bachelor’s degree in Social Work, Psychology, Sociology or related field.
• Proven experience in case management or community support services.
• Strong understanding of social determinants of health.
• Excellent remote communication skills, written and verbal.
• Proficiency with case management software and EHR systems.
• Ability to work independently in a remote setting.
• Reliable internet and suitable home office setup.

Responsabilidades

• Conduct comprehensive remote assessments of client needs.
• Develop and monitor individualized support plans for Richards Bay region.
• Coordinate services with healthcare providers and social agencies.
• Provide ongoing case management and support.
• Maintain accurate electronic case notes and reports.
• Facilitate virtual client meetings and support sessions.
