Qualificações

• Bachelor’s degree in a social service field is required.
• Minimum 2 years in case management or social work preferred.
• Experience with remote client assessments and client advocacy is essential.

Responsabilidades

• Conduct remote assessments and develop individualized support plans.
• Provide ongoing case management with virtual follow-ups and resources.
• Coordinate care with external agencies and healthcare providers.
• Maintain confidential case notes and progress reports per policy.
• Advocate to secure benefits and services for clients.
• Facilitate virtual support groups and psycho-educational resources.
• Monitor progress and adjust plans as needed.
• Respond to inquiries via phone, email, and video calls.
