Placements24 is seeking a dedicated Remote Case Manager to join its Community Support team. This fully remote role serves the Richards Bay, KwaZulu-Natal region, assessing client needs and coordinating services to achieve optimal outcomes from the comfort of home.

You will manage caseloads, develop personalized care plans, and collaborate with health providers, social services, and families while using digital tools to document progress and ensure continuity of care.

Requirements
