Placements24 is seeking a compassionate Case Manager to join a fully remote team serving individuals and families in the Port Elizabeth region. The role involves coordinating services, advocating for clients, and maintaining accurate, confidential records online.

You will work with diverse populations, use virtual tools, and collaborate with internal and external stakeholders to ensure timely access to healthcare, housing and social supports.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• Minimum of 3 years of case management experience in social services.
• Experience with diverse populations and complex needs.
• Proficient with Zoom, Teams and case management software.

Responsibilities

• Conduct thorough client assessments to identify needs and barriers.
• Develop personalized case management plans with clients.
• Coordinate access to healthcare, housing, and social services.
• Provide ongoing support, counseling and advocacy.
• Maintain confidential electronic records and case notes.
• Collaborate with service providers and government agencies remotely.
• Monitor progress and adjust care plans as needed.
• Educate clients about available resources and self-advocacy.
• Participate in virtual team meetings and professional development.
• Ensure compliance with policies and ethical guidelines.
