Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• Experience in case management or similar role within social services.
• Strong written and verbal communication and interpersonal skills.
• Proficient with digital tools, video conferencing, and electronic record-keeping.

Responsibilities

• Conduct comprehensive client assessments to identify needs, strengths, and goals.
• Develop, implement, and monitor individualized case plans in collaboration with clients.
• Connect clients with appropriate community resources, services, and support networks.
• Provide ongoing support, counseling, and advocacy to clients throughout engagement with services.
• Maintain accurate, confidential, and up-to-date electronic client records and case notes.
• Collaborate with other professionals and agencies to ensure coordinated service delivery.
• Regularly review client progress and adjust case plans as necessary.
• Respond to client inquiries and concerns via virtual communication channels.
• Adhere to organizational policies, procedures, and ethical guidelines.
• Participate in virtual team meetings, case reviews, and professional development activities.
