Strong organizational, communication, and problem-solving skills are essential. The role emphasizes client empowerment, advocacy within systems, and collaboration with external agencies.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Master's degree preferred.
• 4+ years of professional experience in case management, social work, or community services.
• Experience in client assessment, care planning, and resource coordination.
• Knowledge of social welfare systems and relevant legislation.

Responsibilities

• Manage a diverse caseload of clients, providing ongoing support and case management services.
• Conduct thorough client assessments via virtual platforms.
• Develop individualized care plans with clients outlining goals and strategies.
• Coordinate access to healthcare, housing, employment, and social programs.
• Maintain regular contact with clients via phone, video, and secure messaging.
• Monitor progress and update case files accurately and timely.
• Advocate for clients within systems and service providers.
• Liaise with external agencies to ensure seamless service delivery.
• Stay updated on relevant legislation and resources.
• Provide crisis intervention and referrals to emergency services when needed.
