Placements24 is seeking a proactive Case Manager to join our community services team in Rustenburg. You will coordinate and deliver essential support services to individuals and families navigating complex social and economic challenges, acting as the primary contact to ensure access to resources and guidance.

You will liaise with partners and agencies, monitor progress, and advocate to help clients achieve personal goals, contributing to the resilience of the local community.

Qualifications

• Relevant qualification in Social Work, Psychology, or a related field.
• Demonstrated experience in case management or a similar client support role.
• In-depth knowledge of social services and community support systems in Rustenburg.

Responsibilities

• Assess client needs and develop individualized case management plans.
• Coordinate and facilitate access to various social services, healthcare, and employment opportunities.
• Monitor client progress regularly and adjust service plans as needed.
• Maintain accurate and up-to-date client records and documentation.
