Job summary

Placements24 is seeking a highly organized and empathetic Remote Case Manager to join our social services team. This fully remote role focuses on assessing client needs, developing action plans, and connecting individuals and families with community resources.

You will maintain confidential records and coordinate services such as housing and healthcare in a remote setting. A bachelor’s degree and at least two years of experience are required.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
• Minimum of 2 years in case management or client-facing support.
• Proven ability to conduct client assessments and develop service plans.
• Excellent virtual communication, empathy, and active listening.

Responsibilities

• Conduct thorough assessments of client needs via virtual channels.
• Develop, implement and monitor individualized service plans.
• Coordinate access to housing, healthcare, employment assistance, and counseling.
• Maintain confidential client records and ensure privacy compliance.
• Communicate with clients via calls, video, and secure messaging to provide ongoing support.
• Liaise with external agencies and community partners to advocate for client needs.
• Prepare reports on client progress and program outcomes.
• Identify barriers to service access and develop mitigation strategies.
• Stay updated on resources, policies, and best practices in social services.
