Job summary
A home care organization is seeking a Home Health Physical Therapist Assistant to provide care for patients in a home setting. The role involves assessing patients’ conditions, performing home visits, and documenting patient progress. Candidates must possess a FL-approved PTA associate's degree and a current FL State PTA license. Reliable transportation and ability to pass a background check are also required. This position is on a per diem, contract basis, offering flexibility in work hours.
Qualifications
- Must be 18 years or older and have successfully completed a FL-approved PTA associate's degree program.
- Must have reliable transportation and proof of auto insurance.
- Must successfully complete a Level II background check prior to offer.
Responsibilities
- Assess patients' condition and perform home health visits.
- Document patient care services and progress/decline in the electronic system.
- Perform therapeutic procedures through manual exercises and mobility aids.

Home Health Physical Therapist Assistant
As a home health Physical Therapist Assistant, your responsibilities will be to assess patients' condition and perform home health visits while following the plan of care developed by the supervising RPT, document patient care services and progress/decline in the electronic system, perform therapeutic procedures through manual exercises, gait, mobility aids, and report patient progress to patient/caregivers/physician/agency office as needed.
- Must be 18 years or older and have successfully completed at least a FL-approved PTA associate's degree program from an industry-accredited institution.
- Must have reliable transportation and proof of auto insurance.
- Must successfully complete a Level II background check prior to offer and initiation of training (https://info.flclearinghouse.com).
- Must complete a competency check and all orientation prior to initiating patient care.
- Current FL State PTA license is required.
All of LAKE, SUMTER, CITRUS AND HERNANDO COUNTIES.
Per diem, Contract (1099) – Part-time, Contract. Pay: Per visit; Varies based on experience.
