Yellowstone Boys and Girls Ranch, Inc. is seeking a Targeted Case Manager to oversee all aspects of a youth’s case and connect families with community resources.

The role provides wrap-around support and requires collaboration with current and prospective services to meet the youth’s complex needs. You will document progress, develop service plans, and coordinate with other providers while maintaining confidentiality and professional boundaries.

Qualifications

• Bachelor’s degree in human services related field required.
• Background clearance per Montana state regulations required.
• Combination of experience and education may substitute for degree.

Responsibilities

• Advocate for youth and families to obtain needed services.
• Maintain contact with service providers to meet youth and family needs.
• Research community services to identify best options.
• Collaborate with youth, family, and professionals to develop treatment plans and lead meetings.
• Complete, update, and maintain required documentation (PQI, intake, progress notes).
• Submit and verify paperwork accurately while ensuring program viability.
