Job summary

Yellowstone Boys and Girls Ranch is seeking a Targeted Case Manager to support youth and families through wrap-around services in the Kalispell area. You will advocate, assess needs, arrange referrals, and help implement individualized service plans in collaboration with schools and community providers.

The role emphasizes confidentiality, professional boundaries, and teamwork with the treatment team to improve outcomes for youth facing complex needs.

Qualifications

• Bachelor’s degree in human services or related field.
• Must pass criminal and child abuse background checks.
• 21 years or older.
• Ability to relate to youth and families therapeutically with confidentiality.

Responsibilities

• Advocate for youth and families and guide them to needed services.
• Assess needs, refer to services, and create/implement service plans.
• Maintain contact with service providers and monitor progress.
• Collaborate with youth, family, and professionals to develop treatment plans.
• Complete and maintain documentation per COA/ARM/Policies and PQI.
• Submit accurate paperwork while supporting program viability.
