Job summary

Yellowstone Boys and Girls Ranch is seeking a Targeted Case Manager to support youth and families by coordinating services and developing comprehensive treatment plans. The role involves collaborating with schools, communities, and professionals to ensure wrap-around care and ensure timely documentation.

Responsibilities include advocating for resources, assessing needs, creating service plans, leading team meetings, and maintaining PQI compliance within COA and Montana ARM guidelines.

Qualifications

• Bachelor’s degree in human services or related field.
• Ability to relate to youth and families therapeutically.
• Criminal/child abuse clearance required.
• Experience working with youth and families in multiple settings.
• Proficient in documentation within EMR and PQI compliance.

Responsibilities

• Advocate for youth and families to obtain needed services.
• Assess needs, refer to services, and create service plans.
• Monitor services and maintain required documentation.
• Collaborate with youth, family, and professionals to develop treatment plans.
• Lead treatment team meetings and coordinate care.
• Maintain PQI and financial viability of the program.
