Yellowstone Boys and Girls Ranch, Inc. is hiring a Targeted Case Manager to coordinate wrap-around services for youth and families in our community. You will assess needs, advocate for services, and collaborate with schools, providers, and families to implement individualized care plans.

Qualifications

• Bachelor's degree in a human services field or equivalent combination of education and experience.
• Ability to relate to youth and families with professional boundaries.
• Proven collaboration skills with treatment teams and service providers.

Responsabilités

• Advocate, support, and guide youth and families to obtain needed services.
• Maintain contact with other service providers to meet youth needs.
• Research services in the community to meet individual needs.
• Develop and lead treatment/service plans with families and professionals.
• Complete documentation within COA, Montana ARM's, Contracts and YBGR policies.
• Prepare and verify paperwork including progress notes and intake forms.
