Job summary

Yellowstone Boys and Girls Ranch is seeking a Targeted Case Manager to provide wrap-around support for youth and families. You will coordinate services, assess needs, refer to resources, and create/execute service plans in coordination with a treatment team.

The role emphasizes collaboration with schools, communities, and service providers to address complex needs, ensure documentation accuracy, and maintain program PQI and financial viability.

Qualifications

• Bachelor’s degree in human services or related field; combination of experience considered.
• Must pass criminal and child abuse background checks per Montana requirements.
• 21 years old or older.

Responsibilities

• Advocate, support, and guide youth and families to obtain services and complete service plans.
• Maintain contact with other service providers to meet youth and family needs.
• Research community services to find the best resources for youth.
• Collaborate with youth, families, and professionals to develop treatment plans and lead team meetings.
• Complete and maintain documentation per COA, Montana ARM's, contracts, and policy.
• Submit and verify progress notes and intake paperwork, ensuring PQI and program viability.
