Job summary

Yellowstone Boys and Girls Ranch, Inc. seeks a dedicated Targeted Case Manager to coordinate wrap-around services for youth and families.

You will connect clients with community resources, assess needs, and develop comprehensive service plans in collaboration with a multidisciplinary team. Responsibilities include documenting care in the required systems, maintaining accuracy of records, and coordinating with other providers to ensure timely access to services.

Qualifications

• Must be 21 years old or older.
• Criminal and child abuse clearance required by the State of Montana.
• Bachelor’s degree in a human services field or a combination of experience and education may be considered.

Responsibilities

• Advocate, support, and guide youth and families in obtaining needed services, creating and monitoring service plans.
• Maintain contact with other service providers to ensure youth and family needs are met.
• Research services to identify best matches for youth and families.
• Collaborate with youth, families, and professionals to develop treatment/service plans and lead meetings.
• Complete, update, and maintain documentation per COA, Montana ARM's, contracts, and YBGR policies.
• Complete and verify paperwork while maintaining PQI and program viability.
