Yellowstone Boys and Girls Ranch seeks a Targeted Case Manager to coordinate wrap-around services for youth and families in Montana. You will assess needs, coordinate with providers, and help create individualized service plans, ensuring accurate documentation and adherence to program policies.

Qualifications include a Bachelor’s degree in human services and the ability to work across settings (school, home, community).

• Bachelor’s degree in human services or related field with relevant experience.

Responsabilidades

• Advocate, support, and guide youth and families to obtain needed services and develop service plans.
• Maintain contact with other service providers to meet youth and family needs.
• Research community services to identify best options for youth and families.
• Collaborate with youth, family and professionals to develop a treatment plan and participate in team meetings.
• Complete, update, and maintain documentation per COA, Montana ARM's, contracts and policy.
• Submit and verify paperwork while supporting PQI and program finances.
