Job summary

Yellowstone Boys and Girls Ranch is seeking a Targeted Case Manager to coordinate wrap-around services for youth and families. You will assess needs, advocate for resources, develop service plans, and monitor outcomes with a collaborative care team.

You will work with schools, providers, and families to tailor treatment plans, document progress, and ensure compliance with COA and YBGR policies. Candidates should be 21+, pass background checks, and hold a bachelor’s degree in a human-services

Qualifications

• 21 years old or older.
• Criminal / Child Abuse Clearance: pass background check as required by Montana.
• Bachelor’s degree in a human services field; equivalent experience considered.

Responsibilities

• Advocate, support, and guide youth and families in obtaining needed services, creating and implementing service plans, and monitoring services.
• Maintain contact with other service providers to meet youth and family needs.
• Research community services to identify those meeting youth and family needs.
• Collaborate with youth, family, and professionals to develop treatment/service plans and lead team meetings.
• Complete, update, and maintain documentation per COA, Montana ARM's, Contracts, and YBGR policies.
• Complete, submit, and verify paperwork including progress notes and intake forms, ensuring PQI and program viability.
