Job summary

Yellowstone Boys and Girls Ranch is hiring a Targeted Case Manager to manage all aspects of a youth’s case and coordinate wrap-around services across Montana. You will advocate for families, develop and monitor service plans, and collaborate with providers to meet complex needs.

Starting salary reflects an 11-step pay schedule, with education and experience shaping placement. Strong documentation skills and use of EMR systems are required; you will work within a multidisciplinary team to support

Qualifications

Bachelor’s degree in human services related field; combination of education and experience may be considered.
Must pass criminal record and child abuse background check as required by Montana.
21 years or older with ability to work in multiple settings (school, home, community).

Responsibilities

Advocate for youth and families to obtain needed services; assess needs and implement service plans.
Maintain contact with service providers to meet youth and family needs.
Research community services to meet unique needs of youth and families.
Collaborate with youth, family, and professionals to develop treatment plans and lead meetings.
Complete and maintain documentation per COA, Montana ARM's, contracts, and YBGR policies.
Prepare, submit, and verify paperwork while ensuring PQI and program viability.
