Job summary

Yellowstone Boys and Girls Ranch is seeking a Targeted Case Manager in Polson, MT. You will manage all aspects of a youth's case and coordinate wrap-around services with families and community resources.

Qualifications include a 4-year degree in a human services field, ability to build rapport with youth and families, and strong documentation and collaboration skills. This is a full-time, on-site position with a focus on service coordination and advocacy.

Qualifications

• Bachelor's degree in human services or related field required.
• Pass criminal record and child abuse registry background check as required by Montana.
• Ability to relate to youth and families therapeutically; maintain confidentiality.

Responsibilities

• Advocate, support, and guide youth and families in obtaining needed services.
• Maintain contact with other service providers to meet youth and family needs.
• Research community services to meet unique needs of youth and families.
• Collaborate with youth, family, and professionals to develop treatment/service plans.
• Complete, update, and maintain documentation in guidelines (COA, Montana ARM's, Contracts).
• Prepare, submit, and verify paperwork while maintaining PQI and program viability.
